我有幸独立承担并圆满完成了我校2005年级259名博士研究生为期三个月的医学英语听说课程的全部教学内容，至今刚过一年。回首这段教学经历，结合学生的学习效果反馈，体会颇深。其中，最让我自豪的是，一个非英语专业的中国教师用全英给博士研究生讲授医学英语，且取得了成功，这在建校史上还是史无前例、开天辟地的事。
一、了解你的“对手”——我校研究生医学英语教学现状
有人对我校96年级研究生医学英语水平进行了一次问卷调查1。结果如下：
公共英语水平考试通过率：；
医学英语水平（自评）:好（）、中（）、差（）；
未修过医学英语：；
英语口语:好（）、中（）、差（）、从未开口说英语（）；
英语听力:好（）、中（）、差（）。
这就是“国情”，是我教学中必须面对而且要认真应对的课题。首先，公共英语水平考试通过率既然高达，这是个唯一鼓舞人心的信息，说明他们的词汇量至少达到了5000～6000左右。这足以满足切远远超过了日常口语交际所需的基本词汇（2000～3000），就看教师采取什么样的方法“诱导”、“激发”学生的潜能了！当然，后面的几条结果更证明了医学英语学习的必要性，虽然有的教授说“博士研究生还有必要学医学英语吗？”有道理，但是永远不要脱离“国情”、“校情”。
怎么办呢？
二、教室——语言学习的“国中之国”
我的几个国外老师和朋友在谈到有关语言学习的诀窍时，大部分都说：WholeEnglish,noChinese。举个大家最熟视无睹的例子，为什么一个从初中开始接触英语并已经学了13年外语的博士生，其口语尴尬得可能不及出国半年的初中生？为什么儿童能够在很短时间内很快听懂并说一口流利的语言？归根到底，简单地说，英语学习就是思维训练的问题。
我的调查发现，许多学生在听完一句英语后的首个反应便是：先在大脑中将片言只语“翻译”成汉语后，再将用汉语进行交流的句子“再翻译”成英语，最后结结巴巴说出来，而且有时还来不及考虑语法问题。也就是将接受到的英语刺激与实物之间,加上两道翻译的手续。交际中的语速、流利程度都受到影响,其根本原因之一,就是没有养成用英语思维的习惯。如此“速率”，如何进行交流？更何况参加国际会议！？
采用全英情景授课，使学生用英语思考，把教室变成学生语言学习的“国中之国”已是迫在眉睫！
虽然许多老师建议我用双语授课，而且我所接触的学生中有相当一部分也希望我用双语授课，但都被我婉拒了。我深深知道，阵痛和血污之后便是新生，冲破黎明前的黑暗便是破晓的晨晖！
我克服阻力，坚定地使用全英授课。事实证明，在反复的语言刺激下，学生对英语作出相应的反应从无到有，由弱到强，从少到多，又被动接收到主动联想，最后脱口而出，形成习惯，进而演变为能力。
培养学生英语思维的能力就是要求学生学习、使用英语时全身心的投入,进人角色。在交际过程中,当英语信息输入后,学生通过英语思维,直接输出英语。
如何在教学实践中实施构建这一“国中之国”呢？
三、精心设计，全方位搭建“国中之国”
首先，大量阅读英文“原著”，培养语感
发给博士生们的教科书从头到尾160万字的正文（均摘自国外原版教科书）内见不到一个汉字！内容涵盖了从基础医学到临床医学、从中医到西医、从结构到功能、从器官系统到分子基因等领域，还提供了大量的医学术语、构词法、病例报告、医疗文书等，加入了最新的有关非典（SARS）、人类基因组计划（HGP）、克隆技术等内容。堪称国内医学英语教科书的“巨著”。每遇到生词，除了自己动手查字典，做标注之外，别无他法。
其次，我推荐给同学们两本全英字典
OxfordLearner’sWordfinderDictionary和LongmanEssentialActivator。这两本字典被誉为discoveringbooks，explosivebooks和dynamicbooks，从活学活用的角度出发，把一个概念用许多种方式联想表达。我以temperature为例说明其用法。
第三，听力材料
摘自英国BBC近年来的力作HumanBody，内容与综合教程相互应，有视频和音频两种格式，约200分钟片长。材料注解后编写成册，设置练习，供课余时间内完成。占考核成绩的40％。还提供了48段完整的临床“接诊－问诊－体检－实验室检查－辅助检查－治疗－病例讨论－出院和随访”全套录音片断.第四，分班、分组，化整为零
我将259名博士分成4个班（分成20个班更好），每班10个组，每组不超过7人。便于提高教学效果，便于学生之间进行交流。小组的表现纳入最终的考核，激发组内每个成员的积极性。
第五，处处留心
告诉同学们随身携带一本小汉英字典和一个MP3，养成随时随地学英语的习惯，看到或听到什么，立即用英语去想该怎么说。我把自拍的白云山风景照片，风景区介绍、群众性体育运动，如蹦极、踢毽子、摩星岭、桃花涧、明珠楼、亭子，甚至路牌（爱护绿化、森林防火等）等给学生看，让他们用英语描述或提出书写错误；禽流感来了,我会谈一点相关知识，之后，先由呼吸病学、传染病学、流行病学的博士从各个侧面介绍背景知识，再由其它学员展开讨论；BarryJMarshall和RobinWarren获得2005年度诺贝尔生理和医学奖后我又让消化病学的博士介绍幽门螺杆菌方面的知识；我还要求学生将我上课用的笔记本电脑的配置用英文陈述；甚至从草地上捡起一朵紫荆花让学生描述……这可能就是所谓的BrainStorm吧。通过者些练习，让学生在所见所闻中“即所得”，养成用英语进行思考的好习惯。
第六，猜谜
有一次我将thalassemia写在黑板上让学生看，绝大部分不解其意。然后我将“地中海贫血”写在一张纸条上“偷偷”递给一位同学，并请他用英文解释给大家听。之后，我会提问其他一些同学说出谜底。如果不知道，则继续请另外一个同学，直到谜底揭晓。有时我会提示一些关键词，比如“anemia（口语：（口语：.）”、“geneticdisease(口语：,)”、Mediterranean（口语：Europe,NorthAfrica,andtheMiddleEast）。
有时我从兜里“偷偷”拿出一粒骰子给一个学生，让他用英语描述，然后让其他学生猜谜。这里我希望学生能够说出以下几个关键词中的2～3个：smallcubes，withsixmarkedfaces，madeofplastic，，forgamesofchance,。
另外，我还允许学生通过手势、动作、甚至画画来表达意思。
爱玩游戏是人的天性，通过“猜谜”，一是活跃了课堂气氛，二是活跃了学生思维，最重要的是，学生明白了：晦涩难懂的医学专业词汇原来可以如此简单地表达！这无疑增强了学习实践口语的信心和兴趣，突破学生英语口语学习的心理障碍。
第七，除了实践，别无他法
采取教师引导，以学生为主体实践对象的教学方法，将课堂实践与考核挂钩。具体做法如下：
每人必须做自我介绍（占考核的10%）；
每组必须完成1个Topic的演讲，并接收提问和答疑，组内的每个成员必须演讲其中的某一个部分（占10%，组平均分即为每个组员的分数）；
课堂提问并回答问题：至少问别人1个问题，至少回答别人提出的1个问题（占10%，积极发言的志愿者可获10%的加分）；
第八，课外活动
语言环境不仅仅在教室内，不仅仅在医学英语课堂上，而应该主动地创造学习环境。这就是为什么我要求他们利用课外时间完成听力练习的真正原因。此外，我还组织了“中－印友谊桥”活动，即每周二晚上将20余名英语发音比较好的留学生与博士们共度“良宵”，在双方互不明白对方语言的情况下创造英语语言环境，并进行交流；我还主持了由JohnPhilip主讲的专题讲座，参与了我校与广州外语外贸大学研究生会举办的英语联谊会等等。

Well, I may be an outstanding doctor in my hospital but I realized two years ago when I was visiting 301 hospital as a trainee, there is a significant gap between those professors and me. The academic atmosphere there also excited me so much that I decided to get further systematic training in famous university.
I took the entry exam twice. The first time, I failed because my English was lower than the level.
